Can a plan created in the Hub be added subsequently to a Microsoft Team?

Yes.

Select this link to view the video version of the instructions given below.

In the Planner Hub:

Open the plan concerned
Select the Three Dots menu
Choose Copy link to plan.

In MS Teams:

Go to the Channel where you want to host the plan
Add a tab
In the Add a tab dialogue, select the Website app.

In the Website app dialogue:

Enter a Tab name (this should be unique)
In the URL box, paste in the link to the Hub plan
Select the Save button.
